How it works
This PTO sign up sheet opens sideways on School and School year lines and 12 lines: Name, Child's grade or teacher, Phone or email, and eight committee check boxes, from Fundraising to Wherever needed.

Check boxes, or two lines a committee
Press Print the sign up sheet for the check box sheet: a parent writes one line and checks every committee they would join. A second sheet prints the eight committees down the side with two lines each. A third has five check columns with empty headings for your own committees.
The rest of the year
A meeting sign-in, a membership list with a Joined check box, room parents by teacher, and seven events to chair: Back-to-school night, Fall festival, Book fair, Winter concert, Science night, Teacher appreciation week and Field day.
- Committees with other names? Use the blank check columns, or cross out a printed name.
Questions and answers
What committees are on the PTO sign up sheet?
Eight: Fundraising, Book fair, Hospitality, Teacher appreciation, Yearbook, Room parent, Field day and Wherever needed. They are common names and not any one school's.
Can another kind of parent group use these sheets?
Yes. Nothing printed on a sheet names the group, so a parent association, a booster club or a home and school club can print the same pages.
Is there a sign up sheet for PTO officers?
Yes. The officer interest list prints President, Vice president, Secretary and Treasurer down the side with two lines each, for a name and a phone number or email.
How is this different from the volunteer sign up sheet?
The volunteer sign up sheet covers one event: jobs, shifts and times. This page covers a parent group's year: committees, meetings, members, room parents and officers. All of it is paper, and the page keeps no names.
