How it works
This daycare sign up sheet is the page by the door. The adult who brings a child writes the time and signs; the adult who leaves with the child does the same on that line, so the open lines are the children still here.

A page a day, or a slip a child
A room of children starts a new page each morning. A home daycare with a few children can keep a week slip for each child instead, so one family's whole week sits on one piece of paper.
If families are signing up for a place
A daycare sign up sheet can also mean the list a family joins to ask for a place. The waiting list sheet is for that, and a family fills it in once, not every day.
Questions and answers
Can I use this sheet for daycare licensing records?
It is a blank sheet of our own, not a form issued by a licensing agency. Ask your licensing office which form it accepts before you rely on it.
Should parents sign with a full signature or initials?
That is your program's choice. The opening sheet leaves room for a full signature. For initials, the daily attendance sheet has two narrow Initials columns and 20 lines.
How many children fit on one daycare sign up sheet?
Fifteen on the opening sheet and 12 on the Picking up today and drop-in sheets, each on one Letter or A4 page. A signature needs more height than a name, so for a bigger room print a second page instead of squeezing in more lines.
