How it works
This free printable sign up sheet opens on Event and Date lines and 20 numbered lines for Name, Phone and Email. Add a line under the sheet makes more, up to 40, and the × on a line takes it off. The Sheet menu has fewer columns, time slots, half sheets and a filled-in sample.

Questions and answers
What should a sign up sheet include?
The event and its date at the top, then one numbered line a person with a name and a way to reach them. The opening sheet asks for a phone number and an email; other sheets ask for one or neither.
Is there a sign up sheet PDF?
Yes. Press the print button and choose Save as PDF as the printer. The file holds the sheet that is on screen, on Letter or A4.
Can I type on the sheet before I print it?
Yes. Press a line, a heading or the Event and Date lines and type; a pasted list of names fills the Name column. An untouched sheet prints blank. The sample in the Sheet menu shows what a finished line looks like.
Can people sign up online with this page?
No. The page prints paper, and people write on it with a pen. What you type on the sheet stays in this browser, so no names, phone numbers or emails are collected.
