How this list was compiled: Our own layout, made for this page on October 9, 2026. Every sheet is a plain form; the headings are everyday school words, and no school district's, publisher's or program's form is copied. The sample lesson and its two teachers are made up. How it works

This co-teaching lesson plan template is for a co-taught class: a classroom teacher with a special education teacher, or a student teacher beside the teacher they are learning from. Every sheet prints on Letter or A4.

Who is Teacher A and who is Teacher B

Write a name on each of the two lines at the top, and the columns then say who does what. Fill each row as a pair: if Teacher A leads the mini-lesson, Teacher B's box says what they do meanwhile. Grouping takes whole class, two halves, stations or a small group aside.

A collaborative lesson plan template for planning together

Co-planning notes has six boxes for the meeting itself, from what worked last week to what each of you does before you meet again. Who does what lists ten classroom jobs, each with a box to check under Teacher A, Teacher B or Both.

  • When one of you leads the whole lesson, One leads, one supports gives the second teacher a job in every part.
  • Splitting the class? Class split in two has a column for the group with each teacher: students, where, task, materials and check.
  • The teacher who is not leading can keep Observation notes: ten students, what you saw and what it tells you both.

Questions and answers

What is a co-teaching lesson plan?

It is one plan for a lesson two teachers give together. Co-teaching, also called team teaching, splits the work between educators who plan, teach and assess the same group of students, generally in one classroom.

Is this a cooperative teaching or collaborative teaching lesson plan template?

Yes, if those are your school's words for two teachers sharing one class. Week for two teachers shows who leads each day and what the other teacher does.

Is there a sample co-teaching lesson plan template?

Yes. The sample fills the opening sheet for two made-up teachers and a math lesson on comparing fractions: both teach the opening and the mini-lesson to the whole class, then each takes one half for practice.

Is there a blank co-teaching lesson plan template PDF?

Yes. Every sheet but the sample is blank, and each saves as its own PDF from the print window, so both teachers can keep a copy of the same plan.
