How it works

This field trip permission slip template prints two slips to a page with a cut line. Each has lines for Trip, Date, Place, Cost and Return this slip by, then the child's name, an emergency contact and a signature line.

Type the trip once

Press a line and type the trip, the date, the place and the cost. What you type on the first slip shows on the second. Then press Print permission slips and cut on the dashed line.

For high school and for your records

The high school field trip permission slip has eight lines where each teacher initials the class a student will miss. The class checklist has a box for each slip that comes back and one for each trip paid.

The slips ask for a contact and a phone number only. If your school needs more, such as health details, use the school's own form.
Set the return date a few school days before the trip, so there is time to ask for the last slips.

Questions and answers

Is this permission slip a legal form?

No. It is a blank sheet of our own, not legal advice and not an official form. If your school or district has its own trip form, that form governs: ask the office which one it accepts before you send this home.

Is there a sign up sheet and permission slip template in one?

Yes. The slip with a chaperone sign up line lets a parent give permission and offer to come along on the same slip. For a list on the door, the field trip sign up sheet has chaperone and driver columns.

How do I save the permission slip as a PDF?

Press Print permission slips and choose Save as PDF as the printer. The page makes no file of its own: the PDF comes from your browser's print window, on Letter or A4 paper.

Can I edit the permission slip before I print it?

Yes. Besides the lines, the title can be typed over, for example to Zoo trip permission slip. Nothing is uploaded: what you type is kept in this browser until you press Start again.
