How it works
This printable student information sheet is one page for a family to fill in during the first week: the student's name and birthday, two parents or guardians, the way home, pick-up, and what the teacher should know.

Make the questions yours
Every question on the sheet can be typed over before you print. Add a line for a question of your own, such as a bus number, or take off one you do not need.
Other forms
The Sheet menu has a form for two households, one that a middle or high school student fills in, half-page cards, a page of open lines, a short form for a club, team or tutor, and a class summary with one row a student.
- Ask only what you will use, and keep the form to one page.
- Copy the answers you need every day onto the class summary, and keep the forms in a folder.
Questions and answers
What should a student information sheet include?
The student's full name, the name they go by and their birthday; a parent or guardian with a phone number and an email; how the child gets home and who may pick them up; and anything the family wants the teacher to know.
Is there a free student information sheet PDF?
Yes, one you make. Press Print the form and choose Save as PDF as the printer. On an iPhone or iPad, tap the share icon on the print screen, then Save to Files.
Is the student information sheet printable on A4 paper?
Yes. Paper size switches between Letter and A4, and each form is one page on either.
Does this replace the school's enrollment or emergency form?
No. This is a teacher's own first-week sheet. The office's forms stay the official record, so ask families to give the office any new phone number, address or pick-up change as well.
