How it works
This behavior frequency data sheet counts how often one behavior is seen: a row for each school day, twelve boxes to cross off and a Total box. Other sheets record intervals, duration and time of day. Nothing is rated.

Print the data sheet
Press Print data sheet. The week tally prints on one sideways page, Letter or A4, with Student, Observer and Behavior counted lines at the top.
Four kinds of record
Frequency sheets count each time. Interval sheets cut one to three hours into blocks of 5, 10 or 15 minutes, each with a Yes and a No box. Duration sheets have start, stop and minutes columns. The time of day grid runs from 8:00 to 3:00 across a week.
What the sheets leave out
Each behavior tracking sheet holds a count, a tick or a clock time and stops there; the page does not say what the numbers mean. The before, behavior and after log is on the ABC behavior chart.
- A boxed row stops at twelve. The four weeks and three behaviors sheets have open boxes for tally marks.
- The last sheet is marked Sample and shows what goes in each column.
Questions and answers
What is a behavior data sheet?
A form for writing down what was seen: how many times, in which blocks of time, or for how long. Frequency is how many times a behavior happens. Duration is the time from its start to its end.
Which behavior data collection sheets are on this page?
Four kinds: frequency tallies by day or by period, interval sheets with Yes and No boxes, duration sheets with start and stop times, and a time of day grid for one week.
Is there a free behavior data collection sheets PDF?
Yes. Press the print button and choose Save as PDF as the printer. The file holds the sheet that is on screen, on Letter or A4.
Is a behavior frequency chart the same as a behavior tracker chart?
On this page, yes. Both names, behavior tracking chart and the British spelling behaviour frequency chart all mean a grid where each time is marked and added up. Behavior data tracking sheets is a name for the whole set.
Is this a behavior tracking sheet for students to fill in?
No. Each behavior tracker sheet here is kept by an adult for one student, whose name goes on the Student line. Nothing is rated, so there are no goals and no points.
