How it works
This ABC behavior chart is a blank observation form with four columns: date and time, antecedent, behavior and consequence. It opens on a sideways page with six rows. Add a row under the sheet gives more, and the Sheet menu has a Setting column, a week chart and single incident records.

Print the form
Press Print ABC chart. The sheet on screen prints on one page, on Letter or A4. For another layout, open the Sheet menu first: an upright page, a Setting column, a week with a row a day, or one incident to a record. Add a row makes ten rows.
What goes in each column
A is the antecedent: what happened just before. B is the behavior: what the observer saw or heard. C is the consequence: whatever happened right after, planned or not. Each time the behavior is seen gets a row of its own.
- Write the Behavior to watch for line first, so every row on the sheet is about the same thing.
- The last sheet in the menu shows what belongs in each column. It is marked Sample and holds no real incident.
Questions and answers
What does ABC stand for on a behavior chart?
Antecedent, behavior, consequence. The antecedent is what happened before the behavior and the consequence is what followed it. The columns run in the order things happened, with one row for each time.
What goes on an ABC chart for behavior?
What an observer saw, written as it happens: the time and place, what came before, the behavior itself and what came after. This page prints the blank form only. It does not say what a record means or what to do next.
Is an ABC behavioral chart the same form?
Yes. It is another name for this record, as are ABC chart behaviour in British spelling and ABC of behavior chart: antecedent, behavior and consequence side by side, here with the date and time in a column of their own.
Can I save the ABC behavior chart printable as a PDF?
Press the print button and choose Save as PDF as the printer. The file holds the sheet that is on screen, on Letter or A4.
