How it works
This personalized learning lesson plan template is for a lesson where students reach the same goal by different routes, each at their own pace. You plan the routes and each student picks one. Every sheet prints on Letter or A4.

One goal, three pathways
Write the goal once, above the table. Each pathway gets a column, with a row for what students do, who might choose it, pace and check-ins, what they need, and how they show the learning.
More personalized learning lesson plan templates
Not ready to open up a whole lesson? One sheet sorts it into what stays the same for everyone and what students choose from. For students there is a playlist of tasks to work down, a six-box plan to fill in with you, and slips for writing down a choice.
- While students work, Check-in notes keeps one row for each short meeting: the pathway, where the student is and what you agreed.
- The class tracker shows on one page who has started, who you have met and who has shown the learning.
Questions and answers
What is a personalized learning lesson plan?
A plan for learning that fits each student. By one published definition, the pace and the approach are matched to each learner's needs. No single definition is agreed, so this template keeps one goal for the class and plain headings.
Is personalized learning the same as differentiated instruction?
Not on these templates. The differentiated instruction lesson plan template plans three levels of one objective, and the teacher places each group. This one plans pathways: a student picks one and moves through it at their own pace.
Is there a personalized learning lesson plan example?
Yes. The sample is a made-up reading lesson on retelling a story in order. One pathway writes the story in three parts, one draws three pictures with captions, and one tells it aloud to a partner.
