How it works
This consequence chart is a blank table of five rows. Each row starts with If I and has then in the middle: the rule goes on the left and what follows on the right, in the adult's own words.

Print the chart
Press Print the chart for five rows on one upright page, Letter or A4. The Sheet menu has three large rows, eight rows, a chart with signature lines and two small charts to a page.
Broken rules, kept rules or both
One set is headed what happens, for a rule that is broken. One is headed what I earn, for a rule that is kept. The both sides chart, sometimes called a reward consequence chart, has three columns: The rule, If I keep it and If I break it.
What the page leaves blank
The page prints no consequence for any behavior: on every sheet the right-hand column is left for the adult. The title line has a blank for a name or a room.
- The three sample sheets fill in example rules only. What follows each rule is left for the adult.
Questions and answers
What is a consequence chart?
A list of rules with what follows each one, written as If I do this, then that happens. This page prints the chart blank, and the adult and child fill in the rows.
Is an if then behavior chart the same as a consequence chart?
Yes. The if then chart is the same table under another name. On the broken rules and kept rules charts each row is already printed with If I at the start and then in the middle.
Is a behavior consequence chart for home or for school?
Either. The headings name no house and no class, and the blank in the title line takes a child's name or a room number. A behavior rules and consequences chart for home and a class chart print from the same sheets.
Does the chart list consequences for me?
No. Consequence charts for behaviour differ from one family and one classroom to the next, so what follows then is left for your own words. Some people call the finished sheet a discipline chart.
